​If you are a new or existing patient and haven't had an appointment with a doctor at Crest Optometry after August 11, 2021, please contact the office to schedule an appointment and have your patient portal activated.  
In your Patient Portal you can:
  • Have access to all your Finalized Glasses, Contact Lens Prescriptions
  • Update your medical history, medications, allergies, family and social history
  • View previous visit notes
  • Message your doctor directly (for urgent matters please call the office)
Please contact the office if you need to:
  •     Update your name, home address or contact information
  •     Reaccess your portal if you get locked out
