In your Patient Portal to the EHR you can:
- Have access to all your finalized glasses and contact lens prescriptions
- Update your medical history, medications, allergies, family and social history
- View previous visit notes
- Message your doctor directly (for urgent matters, please call the office)
- Update your name, home address or contact information
- Reaccess your portal if you get locked out
If you are a new or existing patient and haven't had an appointment with a doctor at Crest Optometry after August 11, 2021, please contact the office to schedule an appointment and have your patient portal activated.
