Crest Optometry is a small private practice that is woman, asian american, and first generation owned since 2011.
Dr. Pham named her private practice “Crest Optometry” after her apartment building in Manhattan while attending optometry school. The Crest was on Wall St and housed quite a few optometry students. It was home for a lot of amazing people and wonderful memories were created. Dr. Pham wanted her private practice to be her second home and a place where one day her colleagues will reunite and work together again.
The office is in a medical plaza off The Alameda. It is just blocks from Whole Foods, the Sap Center, Crema Coffee Roasting Company and Chipotle.

SHIRLEY QUICHO, O.D.
Dr. Quicho was born and raised in San Jose. She graduated from the University of California, Davis with a Bachelor's degree in Biological Sciences. She then went on to receive her Doctorate of Optometry degree in 2012 from Southern California College of Optometry, where she graduated with recognition from Beta Sigma Kappa Optometric Honor Society.
Dr. Quicho is an active member of the Santa Clara County Optometric Society, California Optometric Association and American Optometric Association. She practices full scope optometry and is licensed to diagnose and manage ocular conditions, including dry eye syndrome, eye infections, glaucoma, diabetic retinopathy, cataract and macular degeneration.
Dr. Quicho has been the primary doctor at Blossom Valley Eyecare and associate at Crest Optometry since August 2017. Before Blossom Valley Eyecare, she worked at various private optometry practices in the Bay Area. She is committed to providing the best care possible for all her patients and looks forward to serving you and your family.
During her free time, Dr. Quicho enjoys traveling, trying new restaurants, going to San Jose Sharks games, and spending time with her friends and family.

PHUONG PHAM, O.D.
Dr. Pham grew up in San Jose with a large supportive family. She graduated in 2005 from University of California, Berkeley with a bachelor's degree in Molecular and Cellular Biology. The following fall she moved to Manhattan to attend the four year doctorate program at State University New York College of Optometry. She graduated as a Doctor of Optometry in 2009 with honors. Ever since high school Dr. Pham has always wanted to become an optometrist in her hometown. In 2011 with the help her husband, family, and friends she was able to open Crest Optometry in San Jose. She loves her career and is passionate about caring for her patients. |
Dr. Pham is a proud member of the Santa Clara County Optometric Society, California Optometric Association, and American Optometric Association.

TINA CHANG, O.D., M.S., F.A.A.O.
Dr. Tina Chang, has been working part-time at Crest Optometry and Blossom Valley Eyecare since 2016. She is our ocular disease and TearCare specialist.
Dr. Chang grew up in the Bay Area and went on to complete her undergraduate degree in Biochemistry/Cell Biology at University of California, San Diego. She continued her education at SUNY Optometry and graduated with her Doctorate of Optometry simultaneously with a Masters in Vision Science. She was residency trained in Primary Care & Low Vision Rehabilitation at the Southern Arizona VA Health Care System.
During her residency Dr. Chang gained experience with diagnosis and management of ocular disease, primary care and low vision rehabilitation experience while working at SouthWest Blind Rehabilitation Center. After completing residency she continued working in medical optometry at Naval Hospital Camp Pendleton Department of Ophthalmology where she specialized in pre and post-operative co-management of refractive surgery, ICL, cataract extraction as well as co-management of ocular disease. Dr. Chang is also a fellow of the American Academy of Optometry.
In her free time Dr. Chang enjoys spending time with her family, running and advocating for children with special needs.
